How Do I Use My Benefits?

  • Contact your local ODSP office and ask for a vision care benefit authorization form.

  • Bring your form to Bayview Vision, and show us:

    • your ODSP statement of assistance

    • health card

    • other government ID

  • Pick out your new glasses. Your Doctor will then fill in the form.

  • Sign the form and we take care of the rest by sending the completed form for reimbursement.

    You do not need to pay for eyeglasses up front.
